Fluid – the right amount?

When the Stroke Volume (SV) no longer increases by 10%, the patient is at the top of their Starling Curve and nothing more needs to be done? True or False?

  • Agree
Providing the SV is optimised rather than just ‘normal’ together with other acceptable parameters, the patient can be monitored. However, any reductions in flow may need further consideration.
  • Disagree
Since there is a family of Starling Curves and a patient may move between curves during a clinical episode, it may be valid to add other interventions to optimise SV.

Consider the change in Stroke Volume (SV) following this fluid challenge and whether or not the patient is likely to require more fluid?

  • Give another fluid challenge
SV increased by 13% and another fluid challenge would determine whether the patient is still fluid responsive.
  • No more fluid
Although flow parameters are acceptable, they may not be optimal for this patient. SV increased by 18% and another fluid challenge would determine whether the patient is still fluid responsive.

Consider the change in Stroke Volume (SV) following this fluid challenge and whether or not the patient is likely to require more fluid?

  • Give another fluid challenge
SV only increased by 7% and other flow parameters are acceptable. The high precision of Doppler measurements means that a SV increase of 10% or more is a reliable indicator of fluid responsiveness. The patient can be monitored.
  • No more fluid
SV increased by less than 10% and the other flow parameters are acceptable, so additional fluid is not required at this time.
